seatbelt gadget?

I've seen gadgets that attach to seatbelts and pull the shoulder belt down, so it goes across the shoulder and chest of a short person (instead of digging into his or her neck.) Can anyone tell me what such a thing is called, and where I can find one? Would it work for someone less than 4' tall, or does she need a booster seat?
